Population Health
The population health nurses at each Primary Care Clinic are the main resource for health promotion and disease prevention activities. They manage patient populations and ensure appropriate preventive care services are addressed.
The population health nurses are also part of your health care team and encourage you to schedule an appointment for a preventive screening as many diseases and illnesses can be prevented through early detection and timely healthcare management.

Preventive Screenings Include
Breast Cancer Screening
A mammogram is due every 1-2 years for women ages 40 to 74 years. You do not need to see your primary care manager to schedule a screening mammogram. Please call Womack radiology at 910-907-7966 to make an appointment. You will need to provide the name of your primary care manager to schedule. However, if you have noticed any changes in your breasts, you must meet with your PCM for an evaluation prior to scheduling a mammogram.
Cervical Cancer Screening
A Pap test is due every three years for women ages 21-64 years, or a Pap + HPV test is due every five years for women 30 years and older.
Colon Cancer Screening
All adults, ages 50-75 years, are due for a colon cancer screening. Please speak with your PCM about screening options.
Diabetes A1C Screening
All diabetics, Type 1 and 2, are due for a Hemoglobin A1C lab test every 3, 6, or 12 months depending on your level of control.
