An Army Community Based Medical Home

Community-based medical homes are Army-run primary care clinics located off-post in the communities where Army Families live. But our community-based medical homes offer more than a convenient location. They provide care for over 8,000 enrollees. Community-based medical homes are inspired from the ground up by your ideas and suggestions about how healthcare should be. Those ideas and suggestions are embodied in the concept of the patient-centered medical home.

The patient-centered medical home is both revolutionary and old-fashioned. Each patient partners with a team of healthcare providers physicians, nurses, behavioral health professionals, pharmacists and others to develop a comprehensive, personal healthcare plan. That healthcare team works with each patient over time to take care of health issues as they arise, ensure delivery of prevention screening and services, manage chronic conditions, and promote a spirit of health, wellness and trust. At its heart, the Army community-based medical home is healthcare the way it should be easy to access, patient-centered, team based  and quality focused.

We are committed to providing you convenient access and the right healthcare options to meet the needs of your Family. In addition to your face-to-face visits with your family provider, we offer virtual (enhanced telephone communication) and secure messaging (web-based communication).

These options allow us to follow up on health issues and concerns, share and update health information, and coordinate care delivery. Ask our staff if one of these access options is better suited to fit your needs.

Take NC-87 / NC-24 / North Bragg Boulevard about three miles. Turn right onto Centennial Parkway. Make a U-turn to stay on Centennial Parkway to enter the parking lot of Linden Oaks Medical Home.

From Fayetteville:

Take NC-210 N / Murchison Road toward NC-210 North. Bear right onto NC-87 North / NC-24 West / North Bragg Boulevard. Turn right onto Centennial Parkway. Make a U-turn to stay on Centennial Parkway to enter the parking lot of Linden Oaks Medical Home.

Hours and Appointments

Linden Oaks Medical Home is open from 7:30 a.m. - 4:30 p.m., Monday through Friday. We are closed on federal holidays. Please note that pharmacy and immunizations have additional closed periods (see our “Contact Us” section). We will make every effort to ensure you get an appointment when you want it - including same day appointments. We strive to provide same-day access with your health care team so that you won't have to rely on urgent care centers or the emergency room for your primary care. Well Baby and Child Exams

Pediatric care for children from birth to 18 years of age is available. Children receive a comprehensive examination and immunizations during well child visits.

Internal Behavioral Health Consultation Service

Our Internal Behavioral Health Consultant (IBHC) is available to you in your community-based medical home as a part of your comprehensive health care. The service offers help when stress, worry, or emotional concerns about life problems or medical conditions interfere with your daily life.

The IBHC maybe a psychologist, social worker or psychiatric nurse practitioner specially trained to work as a member of your healthcare team. This team approach allows the team and patients to consider physical, behavioral, and emotional aspects of health.

For example, the IBHC can help you develop plans for smoking cessation or other lifestyle changes. They can also help you with a variety of other concerns such as difficulties with your family or relationship, grief, excess stress, depression, anxiety, or anger.

Minor Surgery

Treatment of boils, abscesses, warts, skin biopsies are examples of just a few minor surgical procedures available. Check with your healthcare team.

Women's Health

Routine appointments for pap smears, well woman exams, gynecological problems, and contraceptive counseling. Women who are pregnant are seen in the OB/GYN Clinic at Womack Army Medical Center (WAMC).

Physicals (School, CYS, Sports) and School Immunizations

Patients assigned to Linden Oaks Medical Home (LOMH) may schedule an appointment by calling the appointment line at WAMC, 910-907-2778 or by calling the clinic directly at 910-908-6257 (OAKS). Please bring all necessary school specific forms with most current shot record and any medical records from network providers.
