Bariatric Surgery Clinic

In accordance with Health Affairs (HA) Policy 07-006, Active Duty Service Members are not eligible for bariatric surgery.

Calculate your BMI
If your Body Mass Index is greater than 40. If your BMI is greater than 35 with significant co-morbidities such as type 2 diabetes, hypertension or hyperlipidemia.

If you meet these criteria, please contact your Primary Care Manager for an appointment to discuss the possibility for surgery and referral.

New Patients

Bariatric Orientation

The Bariatric orientation is the first step towards Bariatric surgery. Our orientation offers a significant amount of education about the Bariatric pathway to surgery along with knowledge of the surgical options we offer at WAMC.

Before attending orientation, you will need a referral from your Primary Care Manager. We will schedule for you an orientation and have you fill out our New Patient Packet.
Please print and fill out this paperwork to its entirety. This is mandatory and we cannot schedule your intake appointment without this information.

Bring this packet with you to orientation. After orientation, you will have your labs drawn and you will be scheduled for an intake appointment with one of our providers!
