Bariatric Surgery Clinic

In accordance with Health Affairs (HA) Policy 07-006, Active Duty Service Members are not eligible for bariatric surgery.

Calculate your BMI
If your Body Mass Index is greater than 40. If your BMI is greater than 35 with significant co-morbidities such as type 2 diabetes, hypertension or hyperlipidemia.

If you meet these criteria, please contact your Primary Care Manager for an appointment to discuss the possibility for surgery and referral.

New Patients

Bariatric Orientation

Congratulations on beginning your journey to Bariatric surgery! Your first step is to watch the orientation video which contains the essential information you will need to navigate the process of having bariatric surgery. Bariatric patients require care from the initial consultation, through the surgery, and beyond. Having bariatric surgery will affect many aspects of your daily life, so we work closely with other members of the healthcare team in our program to ensure that our patients have safe and healthy outcomes. The members of the Bariatric Surgery Team are proud to provide care for patients who are seeking effective treatment for obesity and its associated health conditions.
 
Below are the steps for new patients who have been referred to our clinic. All documents to include your referral must be received by the clinic before the initial intake appointment is scheduled.

Step Two: Download and complete the Certificate of Completion
 
Step Three: Download and complete the New Patient Packet.

Step Four: Attach both documents in an e-mail and send it to the Bariatric Surgery Clinic. Please allow up to 5 business days after your submission to receive a phone call to schedule your intake appointment. 

Please submit your intake packet in PDF format and your orientation certificate in WORD format. We are unable to receive any other format through e-mail. Please do not send any other format other than listed as you will receive an e-mail stating to resubmit. If unable to submit in those formats, please drop off your packet at the front desk of clinic.
