Child-Related Policies
ID Cards for Children
All children 10 years of age and older are required to have a federal Identification Card (ID) card. We will give you a one-time authorization form that you can use for your visit today. Once you have received this authorization you are required to show your child's ID card within 30 days.
Non-Custodial Parent or Step Parent
If you are bringing a child for an appointment and are not the birth parent, you must present a medical power of attorney. This includes the step-parent who is the sponsor of the child. Please see clinic leadership if you have any questions.
