Service Member Behavioral Health

The Womack Service Member Behavioral Health Line has a longstanding reputation for providing quality services and programs to include: evaluation and management of psychotropic medications, individual and group psychotherapy, diagnostic evaluation and psychological testing, neuropsychological services, command evaluations, 24-hour on-call emergency care.

Behavioral Health Evaluations

The Womack Service Member Behavioral Health Line uses Fort Liberty Form 1462 (Request for Mental Status Evaluation) for the following evaluations:
  • Command Directed
  • Schools (Drill Sergeant, Recruiter, etc.)
  • Administrative Separations.

The FB 1462 must be completed by the company commander of the service member being seen. For any questions, please contact your unit's assigned behavioral health clinic.

3rd Special Forces Group Outpatient Behavioral Health

Who: 3rd Special Forces Group, 1st Special Forces Command and USASOAC

What: Provides individual, group, and medication therapy as well as case management. Provides command directed behavioral health evaluations.

East Liberty Outpatient Behavioral Health

Who: 82nd Combat Aviation Brigade, 82nd ABN Division, 1st Theater Sustainment Command, 3rd ESC, EOD/25th Chemical

What: Provides individual, group, and medication therapy as well as case management. Provides command directed behavioral health evaluations.

Robinson Outpatient Behavioral Health

Who: 1st BCT and 3rd BCT and 82nd Sustainment Brigade.

What: Provides individual, group, and medication therapy as well as case management. Provides command directed behavioral health evaluations.

Who: 95th Civil Affairs, 44th Medical Brigade, 525th MI, 16th MP Brigade,
USAJFKSWCS, FORSCOM, 18th ABN Corps, Womack, JFSWCS, JSOC, Air Force (Psychiatry),
CID, Reserves, and all other tenant units

What: Provides individual, group, and medication therapy as well as case management. Provides command directed behavioral Health evaluations.

Clark Outpatient Behavioral Health

Who: 4th & 8th POG, 528th SB USASOC, 108th Air Defense Artillery,
3rd SFG (for SUDCC Only)

What: Provides individual, group, and medication therapy as well as case management. Provides command directed behavioral health evaluations.

Multi-Disciplinary Behavioral Health Clinic

Who: All units not aligned to a specific Outpatient Behavioral Health Clinic and all active duty service members referred for sub-specialty behavioral health services.

What: Provides individual, group, and medication therapy as well as case management. Provides command directed behavioral health evaluations. Provides sub-specialty behavioral health care services to include: neuropsychological assessment, forensic evaluations, and biofeedback services.

Substance Use Disorder Clinical Care (SUDCC)

Who: All service members who meet the requirements and are stationed at Fort Liberty, N.C.

What: Provide Substance Use Disorder Clinical Care, including assessment, treatment, rehabilitation, and aftercare for service members within an integrated medical and behavioral health model to enhance health and readiness.
