Service Member Behavioral Health
The Womack Service Member Behavioral Health Line has a longstanding reputation for providing quality services and programs to include: evaluation and management of psychotropic medications, individual and group psychotherapy, diagnostic evaluation and psychological testing, neuropsychological services, command evaluations, 24-hour on-call emergency care.

Behavioral Health Evaluations
The Womack Service Member Behavioral Health Line uses Fort Bragg Form 1462 (Request for Mental Status Evaluation) for the following evaluations:
- Command Directed
- Schools (Drill Sergeant, Recruiter, etc.)
- Administrative Separations.
The FB 1462 must be completed by the company commander of the service member being seen. For any questions, please contact your unit's assigned behavioral health clinic.

What: Provides individual, group, and medication therapy as well as case management. Provides command directed behavioral health evaluations.
